YOUR FIRST APPOINTMENT:

Please arrive early so that we can complete your initial paperwork. Please bring the following:

* Patient Insurance Card

* List of Current Prescriptions and/or Over-The-Counter Medications

* Information about Patient’s Medical and Surgical History

* Recent Test Results, X-rays, or Relevant Records.

* Driver’s License or Picture Identification

* (For MVAs) A copy of the Police Report

LABORATORY:

Nayles Medical Center is licensed by the State of Arkansas and Clinical Laboratory Improvement Amendments (CLIA), a nationally recognized and accredited organization.

Most tests ordered by your physician will be processed through Quest laboratories. 

We cannot honor prescriptions for lab work from other physicians, such as a specialist you may be seeing.  In those cases, you must go to the reference lab that is designated by your insurance company.

X-RAY:

If necessary x-rays will be performed and read in our office while you wait.

NEW PATIENT FORMS:

Please arrive at least 15 minutes early for your initial visit so that a chart can be prepared for you.  If you have the ability to make a copy of the front and back of your insurance card, please bring these copies with you when you arrive for your appointment.  If not, please be prepared to present these documents when you arrive 15 minutes early for your appointment.
